Report an incident
The council has a legal responsibility to protect the health, safety and well being of all its employees, pupils and visitors. In order to fulfil this responsibility and comply with legislation SCC must ensure that all accidents and incidents are reported, recorded and investigated.

The Health and Safety Management System (HSMS) has been developed to make this process more efficient and less of an administrative burden.

Please note
It is very important that you report the incident under the correct category, therefore please review the descriptions below carefully before selecting the appropriate link.
When completing the incident report, please enter as much information as possible in the details section, ensuring all the information below is provided:
- What was the work activity being undertaken at the time of the accident?
- What happened in the lead up to the accident?
- What were the environmental conditions at the time of the accident e.g. weather, temperature, lighting, floor/ground condition?
- What was the name and type of the machinery/equipment/substances involved in the accident?
- What action have you taken to prevent similar accidents occurring?
- Describe the nature of the injuries sustained
- Has the incident resulted in any time off work being taken and if so, how many days?
- Is there any other relevant information which describes what happened?
Reporting form | Description |
---|---|
Accident/incident | An accident or incident which has resulted in injury to a person |
Violence or abuse | An event or violence or abuse which may include physical injury |
Other | Usually incidents involving asbestos, gas or other dangerous occurrence |
Near-miss or hazard | Incidents which could have resulted in injury, but didn’t or a hazard which is a situation or object which has the potential to cause harm to a person, damage property or the environment |
Reviewing
Head Teachers need to log into the HSMS system using their personal accounts to review accidents and incidents and take actions as requested by CHSS.
