This can also help you have conversations with employers about reasonable adjustments, as well as assisting with getting Access to Work. 

The form is available in a range of formats, including large print and easy read.

Unpaid Carers Support Southampton provide advice and support for people providing unpaid care for someone who is ill or disabled. 

This can include a Carers Assessment that identifies possible support to make things a little easier. They also provide Carers Cards so you can verify that you are a carer and access discounts.

Around 75% of women will experience menopause symptoms; for 25% of women they will be severe and they can have a big impact on daily life, including employment.
